Letter of Acknowledgement

Before you proceed with your PCR Test and/or Serology test below, please ensure that you have read and understood all requirements:

 

Documentation required:

1. NRIC and Passport

2. Details of your flight booking with departure date and time (hard or soft copy)

3. Handphone, Email Address and Home Address

 

If you have frequent nose bleeds, recent surgery or trauma to your nose, a very crooked nose or nasopharyngeal cancer we will not be able to do the Swab Test at our clinic. Please ensure you check your destination countries requirements as age limits for children may vary.

Children below 12 years of age will have the Pre-Departure COVID-19 PCR Test done.

Pre-Departure COVID-19 PCR Swab Test result is positive:

If the Pre-Departure COVID-19 PCR Swab Test result is positive, we will not be able to issue a memo for fitness to travel. You are not permitted to Travel.
 

Pre-Departure COVID-19 PCR Swab Test results may return as inconclusive:

In that event, a repeat test may be conducted and is chargeable. Should this result affect your departure time, Etern Medical Clinic and/or the appointed laboratories vendor(s) shall not be held responsible or liable for any claims, damages, losses, expenses, costs or liabilities.

 

Delivery of Results

We will email your lab results and memo to you as soon as it is available. If the result is positive or inconclusive, we will contact you via phone or text messages. You are advised to print out and bring along the hardcopies of your results with you overseas.

 

By Signing this agreement, I agree to the following conditions set out:

1. I have checked the timeframe requirements of the destination countries I am travelling to, the travel lanes and the PCT testing window frame. I will not hold Etern Medical and the laboratory liable for any additional costs incurred should there be any rejections from the airlines, embassy and authorities if I have made the wrong appointment based on the country’s pre-arrival swab requirement time.

2. I acknowledge that Etern Medical Clinic accepts no responsibility or liability for any missed flights due to inconclusive results or any other unpredictable factors that may occur.

3. I understand that if my pre-departure test result returns as positive, I am not permitted to travel and MOH will be notified of my results.

4. I agree that Etern Medical may collect, use and disclose my personal data to contact me in accordance with the Personal Data Protection Act (PDPA) 2012 and Etern Medical’s data protection policy.
