Please bring your Identification Documents and the Medical Form on the day of the appointment. For Renewal of License, Individuals are required to go for the medical checkup: For 50 to 64 years: Every 2 years For 65 years and above: Every year For Crane Operators aged 70 years and above, you must go for additional tests: Abbreviated mental test – to take together with the medical examination. Clinical occupational therapist assessment – to be taken after passing the medical examination.
