Terms and Conditions - Lasting Power of Attorney (LPA) & Advance Medical Directive (AMD)
Etern Medical provides a medical consultation and certification service for your Advance Medical Directive (AMD) and Lasting Power of Attorney (LPA) forms. Our role is to ensure that you, as the patient, are of sound mind to make these important decisions and that you understand the medical implications. Please note that we are a medical service provider, not a legal firm, and we do not provide legal advice. For legal advice regarding your estate or specific legal questions, we strongly recommend you consult a legal professional.
Important Notes Before You Book:

Age Requirement: The person making the AMD or LPA must be at least 21 years old.​

LPA: Only book your appointment after you complete your form application & donee(s) appointment.

AMD: You may obtain the application form here, or inform our clinic teams to get the form ready for you during the consultation.

Witnesses: The law requires your forms to be signed in the presence of qualified witnesses. For both AMD and LPA, please ensure you bring your two witnesses with you to the appointment. Our doctors can also serve as one of the two witnesses for your AMD form.

Sound Mind: You must be of sound mind to complete these documents. Our doctor will need to verify this during your consultation.

For Singaporeans & Permanent Residents: Ensure that you and your donee(s)'s SingPass is ready for verification.

For Foreigners: Ensure your passport or any form of personal documentation is ready for verification.
Comprehensive Plan
If you wish to complete both your LPA and AMD, please inform our clinic team. We can schedule a single comprehensive consultation to complete both documents in one visit with our certified doctor.

The appointment timing is an estimated timing, but we will do our best to adhere to your chosen timing.

This is not a confirmed booking, our staff will contact you to confirm your eligibility.
During Booking

Upon booking, a confirmation email will be sent to you. This email can only be used for the service indicated.
Reschedule Booking

If you are unavailable, please contact the respective clinics to reschedule your appointment.
Appointment Day

Please arrive 10 minutes before your appointment for registration. You will need to present the confirmation email upon registration at the clinic, together with your NRIC or other identification documents.

Both Donor and Donee(s) are required to be present for the appointment.

Prepare all pre-requisite LPA documentation and ensure that both Donor and Donee(s) are able to log into SingPass.

All payments will be made in the clinic.
